The health care industry is a major contributor to the problem systemically. Many of the most serious problems are institutional and cultural.

A reading of the article shows that the culprit is "capitalism" understood as favoring capital accumulation. This results from asymmetrical power. The consequence is bias against social purpose.

For example, the assumption that firms are operated to maximize "shareholder value rather than stakeholder value results in prioritizing efficiency and "cost-effectiveness" (relative to management compensation and the bottom line) over stakeholder value, whereas the entire society is the major stakeholder in its health care delivery system.
