Cock-ups are a hundred times more common than conspiracies. The Arab League warned Bush, Blair etc that invading Iraq would in the words of the Arab League "Open the gates of Hell". It did, in the form of ISIS. B&B didn't listen. Libya was the same on a smaller scale.
